Your return home
On leaving the hospital, we arrange with you the initial meet and greet visit where we will establish exactly what type of support you require. With your consent, we will together complete a detailed assessment and refer you to other health, social or voluntary sector organisations, should you require them for ongoing support. Our role is to support you on discharge from hospital to recovery. Our nurse led PNAs will work closely with you following your own Personal Support Plan, and ensuring choice and control are paramount at all times in making decisions about your care.
Patient confidentiality
Hilton Patients can be confident that: Their personal records, including medical records, are accurate, fit for purpose, held securely and remain confidential. Other records required to be kept to protect their safety and well being are maintained and held securely where required. Sometimes, there are circumstances where confidentiality needs to be breached. If an illegal act has taken place or harm is intended, then the manager has a legal responsibility to disclose information to Safeguard. Reasons would be given if the person could not give consent to the disclosure. Patients have the right to access information held in any format, which relates to them. We do not provide services to anyone under 18 years of age.
