We believe that every person deserves the opportunity to come home to recover and be supported in their own homes. We know that leaving hospital and returning home can be a daunting prospect . You can rely on your local Hilton Nurse led team to provide you with the support, planning and nursing care you need until you’re back on your feet again. Our 3-5 day recovery service provides you with a tailored programme of support to meet your individual needs in those first few days after returning home.

With a Hilton Personal Nursing Assistant, you can recover more quickly in the comfort and security of your own home. That’s one of the reasons hundreds of people rely on us every day to help them recover after illness, accident or a stay in hospital.

Working with the NHS and Social Care Commissioners we successfully deliver safe, timely and supportive hospital discharges, patient assessments and patient recovery programmes. Our nurse led partners are on hand whenever you may need us and are only ever a phone call away. We take a collaborative approach to your needs, involving your family members, friends and multidisciplinary teams to complete your Individual Support Plan if appropriate.
