We are looking for enthusiastic and caring individuals to work within this service, and as an Assessor will need to have had experience of completing assessments in a Health or Social Care setting. You will be working in a team of Assessors and Personal Nursing Assistants (PNA). It will be an exciting new challenge where each day is different, and your actions have a big impact on people’s lives.
We support patients at home following discharge using a model of person centred care, the patients are then assessed for their future needs. We work proactively with other ‘partners’ (the Hilton team, patient’s family and friends, community services and other professionals) to promote independence and wellbeing in all aspects of their lives. Your role will be to complete accurate and comprehensive assessments of a patient’s needs and identify which health and social care pathway is required. You will need to work closely with other professionals and organisations, building relationships and knowledge of services.

- Is dedicated to provide the right care for each individual patient, ensuring the patient is central to the assessment process.
- Has excellent communication and documentation skills.
- Has a professional manner and adheres to the code of conduct for Healthcare Support Workers.
- Is able to quickly build trusting relationships with patients, families and other professionals.
- Enjoys working within a strong team but is also confident working independently.
- Is calm and good at problem solving, and happy to ask for support and advice.
- Is organised and able to plan for the day considering changing priorities.
- Is highly motivated and enthusiastic with a willingness to learn and develop to meet challenges.
- Understands the need to be flexible and adaptable to the changing needs of the organisation.
- Needs to have a current driving licence and own car
