Hospital discharge for the frail requires much more integration between Health & Social Care to ensure better recovery. Now in this time of COVID-19 its even more critical that the right care package is offered to anyone leaving hospital to ensure they not only avoid exposure, but do not transmit the disease.

It’s saddening to read that a ‘third of frail hospital leavers in England find post-discharge care lacking’. The reality is that through better integration of Health and Social Care post-discharge care could be vastly improved without draining resources or adding to costs. With the right guidance and support from a company like Hilton, post discharge care can be provided in a patient’s home through continous and carefully managed support.

We have found discharge care in a patient’s own home leads to a hugely positive recovery experience, allowing time and space to make those  ‘next step’ care package decisions should they be required.
