Hilton Case Study – Miss L

Miss L had spent some time in hospital due to a number of falls. She had a good family support network but wanted to remain independent in her own home.

Miss L was living with mild dementia and diabetes. Hilton discharge to assess took place and on the second day home, Miss L fell again in the early morning. The Ambulance had been called and they tested her glucose level which was extremely high.

Together the Hilton team and paramedic with family input, agreed a plan to bring down the glucose level and monitor Miss L constantly until she became stable once again.

Miss L remained at home throughout this episode and was not re-admitted to hospital. At the end of the recovery period, Hilton signposted the family to identify suitable private care arrangements in addition to community organisations to assist with shopping and befriending services, which suited Miss L’s lifestyle and care requirements.

Miss L experienced a seamless transfer to a private care provider at the end of the Hilton recovery programme.
