Hilton Case Study – Mr M
Patient Mr M was admitted into Hospital after having a fall while he was out walking his dog. Whilst in hospital Mr M became confused, especially at night. Causing him to become aggressive, delusional and experiencing hallucinations. Mr M underwent an inconclusive dementia assessment, but health professionals suggested a follow up assessment as an outpatient once he returned home.
Mr M was referred to Hilton discharge and recovery service. Hilton commenced the meet and greet and community team introduction visit immediately on his return home. The Hilton PNA team carried out a home risk assessment and devised a five-day recovery plan with Mr M and his daughter to support him back to full independence by supporting and monitoring Mr M’s safety and well-being at home, medication, nutrition and fluid intake, and personal hygiene.
Mr M expressed to his Hilton PNA’s that he felt it may be hard to settle back at home as he had been in hospital for three weeks. But with Initial support identified as three times a day on the first two days, Mr M began his Hilton recovery experience. Mr M initially struggled with his new dosset box and change of medication, but soon become independent of support once familiar with the meds and the box. He was able to shower himself with support, independently changed his clothes and was very well presented! He was observed making his own sandwich lunch and cup of tea and on the third day cooked a hot pie dinner with vegetables. The support was reduced on day three to two calls per day and on day five Mr M was out walking his dog. Hilton discharged Mr M from the service on day five as fully independent of any formal ongoing support. Mr M re-engaged back into his own family and community support networks and his outpatient appointment was organised via his GP.
