Mrs A was brought into A&E on a Saturday morning, after an ambulance was called following a fall by her front door. Mrs A had been in A&E for a number of hours and was discussed throughout the day by health teams as a ‘Social’ referral as Mrs A was ‘wandering’.

Hilton discharge lead was asked to assess Mrs A at 15:45 by the Mental Health Social Worker, as she considered that although Mrs A was a little confused she was not suffering from dementia. Hilton spoke with Mrs A and phoned the neighbour who held the keys to her home.

Mrs A was a little confused as she had not eaten since she arrived in hospital. She was well – presented and eager to return home to her dog. With an extremely sore hip due to the fall, Mrs A needed to keep walking to relieve the pain.

Hilton arranged transport and a meet & greet visit on her return home by a PNA team. Mrs A left A&E at 16:20. After only two days of Hilton support Mrs A completely recovered and was out shopping with friends by Monday. A true Hilton Success.
