Home to Decide reduces unnecessary care home admissions, saving money for commissioners and providing a better quality of life to those on the service. The service lasts up to 14 days starting at an intensive level and reducing in line with the individual’s progress.
To date, an amazing 67% of people completing the service have been able to continue to live at home. Our confidence in delivering outcomes enables us to share the financial risk.
The cost of the individualised one-to-one service will be capped at the residential care weekly rate. If the person remains at home, savings are shared for 10 weeks, if they enter residential care, there is no further cost.
